For a minor's tickets a parent or guardian must be responsible for agreeing to the following terms and conditions. If this ticket is for a minor the parent is required to include their name as a signature in the following prompt.

WAIVER AND RELEASE OF LIABILITY
By purchasing a ticket to the Latina History Day conference for myself or others, I confirm that the following shall apply:

I/my guest:
• have chosen to voluntarily participate in the activities/events listed above ("Latina History Day 2025”) by Hispanas Organized for Political Equality ("HOPE").
• have not been in close personal contact, meaning within six (6) feet and for at least ten (10) minutes of someone at work or outside of work with suspected or confirmed COVID19 in the last fourteen (14) days without wearing the proper personal protective equipment.
• have not experiencing any of the following symptoms: new or unexplained cough, congestion, or runny nose, new or unexplained or worsening shortness of breath, sore throat, new loss of taste and smell, fever or chills, new unexplained muscle or body aches or fatigue, headaches, nausea, vomiting or diarrhea.
• acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while attending the Event and that such exposure or infection may result in personal injury, illness, permanent disability, and death.
• understand that HOPE cannot be held liable for any exposure to the COVID-19 virus caused by misinformation on this form or the health history provided by each participant.
• hereby indemnify, release, discharge, and hold harmless HOPE, and their respective successors, licensees, employees, officers, directors, agents, and assigns, from any and all claims, actions, costs, expenses, judgments, liabilities, damages, losses, and demands of any and every kind and character and under any and all theories of law or equity (including attorneys' fees), and from any and all liability for any loss of property, damage or personal injury of any kind, nature or description, that may arise or be sustained by me, or by any third party, during or related to my attendance at the Event. This release will be binding upon my/our heirs, administrators, executors, and assigns.
